1. SATUSEHAT Health Pass (SSHP) Form: Required: Effective as of August 27th, 2024 and applies to all international arrivals who are entering Indonesia.

2. Place of arrival: The traveler must arrive and depart from one of the following airports:
3. Details: Required to submit details such as full name, phone number, and passport number. The form also requires some flight details, such as date and port of arrival (in the case of Bali Airport is DPS), as well as flight and seat numbers. EN and JP

8. When to be submitted: SSHP must be completed within 48 hours of departure for Indonesia.
9. QR Code: Upon completing and submitting the information, a QR code will be generated for presention to the health officials upon arrival. Taking a screenshot, saved as a link or save the image in your phone’s gallery.
