1.	SATUSEHAT Health Pass (SSHP) Form: Required: Effective as of August 27th, 2024 and applies to all international arrivals who are entering Indonesia. Available by clicking SSHP EN Alternatively, available by clicking below either the button for Sign in or Sign up EN and JP
2.	Place of arrival: The traveler must arrive and depart from one of the following airports:
3.	Details: Required to submit details such as full name, phone number, and passport number. The form also requires some flight details, such as date and port of arrival (in the case of Bali Airport is DPS), as well as flight and seat numbers. EN and JP
4.	Passport: The arriving traveler's passport is to be valid for at least six months from the departure date from Indonesia; Uploaded here by clicking either the button for Sign in or Sign up.
5.	Proof of departure: Outbound/return e-ticket; Copy uploaded here prior to arrial by clicking either the button for Sign in or Sign up
6.	Stay duration: 3 days up to 30 days max.
7.	Stay extension: extendable for another 30 days within Indonesia
8.	When to be submitted: SSHP must be completed within 48 hours of departure for Indonesia.
9.	QR Code: Upon completing and submitting the information, a QR code will be generated for presention to the health officials upon arrival.
